Uninsured Services
The Proactive Plan offers services not covered by Alberta Health Care, including: proactive and preventative health advice by allied health practitioners in person, by phone, by mail or by electronic mail; in-depth nutrition and diet counseling, exercise, fitness and lifestyle advice, therapeutic massage and psychological counseling or any other consultations regarding health and wellness with members of our team that are not covered by the Alberta Health Care Insurance Plan. The annual fee is for services performed by our health practitioners and medical services not covered by your Alberta Health Insurance.
Insured Services
Services that are medically required or indicated, and covered by Alberta Health Care, will be provided by the Provital Family Physicians and billed to the Alberta Health Care Insurance Plan
*According to the Alberta Medical Association Guidelines the following are Uninsured Services and are not covered by the Alberta Health Care Insurance Plan. These services are included in the annual fee with Provital Health and Wellness:
- Consultant review of appropriate medical records and patient data with subsequent written opinion to referring physician, when patient not seen directly
- Preventative medical counseling
- Advice by telephone (telemedicine) – According to the CPSA standards of Practice
- Renewal of prescription by telephone
Comprehensive Health Assessment
*The Provital Physicians bill all insured portions of this assessment to the Alberta Health Care Insurance Plan. Portions of the assessment that are completed by our allied health practitioners are covered by the annual fee paid with the Proactive or Platinum plan

Tax Deductible Services
Specific medical services performed by physicians and alternate practitioners may qualify as a tax deductible medical expense under the Canada Revenue Agency Legislation. An itemized and specific invoice will be maintained for each patient, and will outline specific services qualifying for tax deduction.
